Job Details

*This is a MOBILE position. Employee starts from home and travels to patient homes/visit locations.

Schedule:
24 hours per 2 weeks
-Primary shift 5p-8p (at least 2 shifts/week)

-Every 6th weekend:
Sat and Sun from 8a-4:30p
On call shifts will be Fri, Sat, Sun from 6p-10p

RESPONSIBILITIES:
The RN Home Infusion is a licensed professional who plans for, coordinates, and provides individualized nursing care to patients and families in their homes. The provision of home infusion services reflects the mission, philosophy and values of the agency. This position reports directly to the Care Team Leader or Nurse Manager of the Department.
- Interacts with patients, co-workers, and other staff, consistent with the values of Jefferson.
-Performs patient care in a timely and efficient manner.
-Meets productivity expectations.
-Maintains a positive impact on customers (internal and external)
-Implements, reviews and revises plans of treatment.
-Performs patient assessments as required by patient condition and or by agency policy.
-Provides client education for clients/caregivers receiving IV therapy at home
-Initiates, maintains, and discontinues IV services with clients in a complete, thorough and timely manner.
-Submits documentation within the 48 hour guideline
-Communicates with other members of the health care team on issues pertaining to client care needs, status updates, and coordination of care issues.

Job Description

Minimum Education and Experience Requirements:

EDUCATION: Graduate of accredited nursing school

EXPERIENCE: At least 2 years in the acute care setting or 1 year in home care, hospice or home infusion setting

Minimum Certifications, Registration or License Requirements:

Current RN licensure in state of employment; Provider in Basic Cardiac Life Support; valid driver’s license.
