Job Details

Job Summary :  The Nurse Manager, Care Management oversees the day-to-day operations of the Care Management Department.  The Nurse Manager, Care Management provides support and guidance to the Care Management staff; balancing clinical, strategic, regulatory, risk management and financial aspects in everyday decisions about care coordination, resource utilization and discharge planning.  The Nurse Manager, Care Management role also encompasses education of the entire interdisciplinary team on the goals of the Care Management Department. The Nurse Manager, Care Management works collaboratively with other departments to achieve positive outcomes and a positive patient experience for all St. Catherine’s patients and families.

Responsibilities

  • Establishes, manages, and evaluates the day-to-day operations of the Care Management Department, ensuring effective and efficient delivery of services.
  • Establishes clear roles, responsibilities, and performance expectations for Care Management staff.
  • Develops and maintains staffing schedules and manages time-off requests to support appropriate departmental coverage.
  • Provides ongoing coaching, constructive feedback, recognition, and support to Care Managers to promote performance excellence and professional growth.
  • Monitors employee performance and participates in annual performance evaluations.
  • Ensures timely, accurate, and appropriate documentation of Care Manager interventions and activities.
  • Participates in the orientation and onboarding of newly hired Care Management staff.
  • Participates in daily Escalation Huddles to identify and address delays, barriers to discharge, and other factors impacting patient throughput.
  • Collaborates with the Physician Advisor, attending physicians, and interdisciplinary teams regarding clinical and utilization management issues, as appropriate.
  • Reviews patient level of care for ambulatory, observation, and inpatient admissions and collaborates with the attending physician and clinical team to resolve identified issues.
  • Communicates changes in patient level of care status to Care Managers based on physician orders and/or updated clinical information.
  • Serves as a resource to Care Management staff regarding regulatory requirements, managed care guidelines, medical necessity, and utilization management practices.
  • Collaborates with the interdisciplinary team to ensure medical necessity, appropriate patient status, timely progression of care, and optimal patient outcomes.
  • Facilitates patient throughput and supports effective transitions across the continuum of care.
  • Monitors and participates in initiatives designed to reduce avoidable delays and length of stay (LOS).
  • Provides ongoing education, professional development, and training for Care Management staff, both individually and as a group, and monitors staff engagement and satisfaction.
  • Serves as a clinical and operational resource to Care Management staff, physicians, and hospital personnel.
  • Promotes effective collaboration among Care Management, clinical services, and other hospital departments to optimize patient outcomes and the patient experience.
  • Represents the Care Management Department on interdisciplinary teams, committees, and organizational initiatives.
  • Participates in committee work at SCS and contributes to the development and implementation of new programs and initiatives as assigned by the Director of Care Management.
  • Identifies opportunities for process improvement and supports initiatives that enhance Care Management operations, quality, efficiency, and patient outcomes.

Minimum Qualifications

  • Graduate of an accredited School of Nursing with a current New York State Registered Nurse (RN) license required.
  • BSN or Bachelor's degree in a healthcare-related field preferred, Master Degree in related field preferred.
  • Experience in acute care Care Management.
  • Approved National Certification preferred.
  • Management experience in Care management preferred.


Salary Range

USD $128,000.00 - USD $160,000.00 /Yr.



This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
