These roles are not just about vitals; they are about integrated care—treating the "whole person" by bridging the gap between physical and mental health. The CCBHC Nurse is a vital member of a multidisciplinary team dedicated to providing integrated, community-based care. You will work alongside therapists, psychiatrists, and case managers to provide health screenings, medication management, and chronic disease education to individuals with serious mental illness (SMI), substance use disorders (SUD), and co-occurring physical health conditions. QualificationsSummary of Qualifications  Graduate of an accredited nursing program        Current and active Vermont RN license  At least 1 – 2 years of experience in behavioral health and, or community health settings is preferred The ability to deliver services and complete treatment notes in English Workplace Conditions The position will coordinate care for children and adults, from a variety of backgrounds, in face paced and, at times, unpredictable environment.
