JOB SUMMARY
This position serves clients primarily in the Elkhart County region.
This full-time, at-will position connects individuals and families with programs, services, and supports to
promote independence and well-being. This position provides guidance on available resources, assists with
applications and referrals, and collaborates with staff, caregivers, and community partners to ensure timely and
effective service delivery.
ESSENTIAL DUTIES
- Assist individuals and families in identifying, navigating and accessing community resources, programs,and benefits to support their needs and goals.
- Conduct outreach, home visits, and comprehensive assessments to evaluate individual needs anddetermine eligibility for CHOICE, SSBG, TITLE IIIB (Older American Act).
- Provide guidance and support with applications, documentation, and referrals to ensure individualsreceive timely and appropriate access to services.
- Maintain accurate records of all activities, interactions, and outcomes, ensuring tasks are completed in atimely and precise manner.
- Collaborate with internal staff, caregivers, community agencies, and service providers to coordinate careand facilitate continuity of services.
- Develop initial service plans for Health and Wellness waiver, Traumatic Brain Injury waiver, PathwaysHCBS Fee-for Service, and Money Follows the Person, ensuring compliance with programrequirements.
- Identify and engage enlivIN participants; develop individualized service plans addressing socialdeterminants of health.
- Provide telephone reassurance to individuals participating in waiver programs or on waiver waiting list.
- May provide services and support across multiple programs and/or projects, adapting to organizationalpriorities while maintaining the core functions as described.
- Other duties may be assigned and/or requested from time to time in fulfillment of the agency’s mission.
RESPONSIBILITIES
The Community Resource Specialist assists individuals and families in accessing Medicaid and other
community resources by providing guidance, conducting visits, and completing accurate documentation. The
role includes monitoring applications, coordinating with staff and external agencies, and supporting clients
through referrals and problem resolution. Additionally, the position participates in training and projects,
maintains required systems, and travels within the service area to meet program needs. Employee will perform
their duties in accordance with the Allowable and Unallowable Activities for the service provided, specifically,
the Care Management section of the most recent version of the Indiana Health Coverage Programs, Provider
Reference Manual, Home and Community Based Services Waivers (“Waiver Manual”).
Requirements
EDUCATION AND/OR EXPERIENCE REQUIREMENTS
-  Bachelor’s degree (BA or BS) from an accredited four-year college; or,
-  An associate’s degree may be considered with four or more years’ experience in human services, assessment, service plan development and implementation.
- Â Obtain and maintain ongoing Care Manager Certification with the Indiana FSSA.
KNOWLEDGE, SKILLS, ANDABILITIES REQUIRED
- Â Strong knowledge of community resources and public assistance programs.
- Â Excellent verbal and written communication skills; ability to conduct interviews and explain complex information.
- Â Ability to plan, organize, prioritize, and track multiple tasks and responsibilities.
- Â Ability to learn and effectively use specialized information and referral databases.

Work requires balancing multiple priorities, accurately completing documentation, and ensuring compliance
with established procedures and regulations. Duties involve interpreting program requirements, exercising
sound judgment, and coordinating with individuals, families, and external agencies to address diverse needs.
The role also requires adaptability, problem-solving, and the ability to learn and apply specialized processes.

CONDITIONS OF EMPLOYMENT

-  Must meet the Agency’s required car insurance coverage level at employment and maintain it throughout the duration of employment (Liability Coverage: $100,000/$300,000/$100,000).
