We are looking for experienced Psychiatric Mental Health Nurse Practitioners, PMHNPs for “SNF-ists,” in-house nurse practitioners that will provide care for the geriatric population in a Skilled Nursing and/or Assisted Living setting.

Primary Responsibilities:

  • Act as a Skilled Nursing Facility’s leading provider of internal medicine services.
  • Collaborate with the facility’s interdisciplinary team and physicians.
  • Serve as a day-to-day clinical leader while helping oversee the care of all residents.
  • Work with the entire facility’s patient population - short and long-term residents.
  • Prescribe all necessary medications and treatments.
  • Provide monthly stability visits for long term residents.
  • Perform comprehensive physical examinations of patients.
  • Develop and implement patient management policies and procedures.
  • Facilitate referrals to other healthcare professionals and medical facilities.
  • Communicate with attending physicians and/or specialists regarding patient care.
  • Be comfortable and proficient with documentation and CPT coding.
  • Provide evaluations for patients exhibiting changes in condition.
  • Advanced care planning.
  • Oversee residents day-to-day care with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians.
  • Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease.

Requirements:.

  • Master’s Degree Required
  • Must hold certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP) in the state of North Carolina
  • Must have an active license and DEA #
  • Previous experience in nursing or other medical fields
  • Minimum one-year experience in a Skilled Nursing setting (preferred)
  • Ability to build rapport with patients and their loved ones.
  • Ability to thrive in a fast-paced environment.
  • Excellent written and verbal communication skills
  • Strong leadership experience with emphasis on teamwork in a healthcare setting
