Position Details:

  • Main Office location: Worcester, MA
  • Service Area: Spencer and surrounding areas.
  • Job Type: Full Time
  • Schedule: Monday to Friday

Responsibilities:

  • Provides skilled nursing care to patients in their homes under the direction and supervision of a Registered Nurse and in accordance with the established plan of care.
  • Monitors the patient’s condition and collects relevant health information during each visit, reporting changes in condition or needs to the supervising Registered Nurse and other appropriate members of the care team.
  • Contributes to the development and modification of the plan of care by providing observations and updates regarding the patient’s condition, response to treatment, and ongoing needs.
  • Performs appropriate preventive and rehabilitative nursing procedures in accordance with the established plan of care.
  • Administers medications and treatments as prescribed by the physician or other authorized provider and in accordance with the plan of care.
  • Observes and documents the patient’s response to medications, treatments, and interventions and promptly reports significant changes or concerns.
  • Completes, maintains, and submits accurate and timely clinical documentation regarding the patient’s condition, care provided, and response to treatment.
  • Communicates with the supervising Registered Nurse, physician, and other members of the care team regarding patient needs and changes in condition, in accordance with agency policies and applicable scope-of-practice requirements.
  • Provides education and reinforcement to patients and caregivers regarding medications, treatments, safety precautions, and other aspects of the established plan of care.
  • Promotes and maintains a safe environment for the patient in the home.
  • Participates in interdisciplinary team meetings as appropriate.


Qualifications:

  • 2+ years' nursing experience, preferably in geriatrics is desirable.
  • New nurses, feel free to apply!  We have a great training program.
  • Degree in Nursing from an accredited institution.
  • Proficient with electronic medical records.
  • Excellent bedside manner and communication skills.
  • Working knowledge of health care laws and regulations.
  • Maintain current BLS CPR certification.
  • Possess a valid driver's license and own a reliable, insured vehicle.
