Care Manager RN, Providence St. Lukes Rehabilitation Inst, Spokane WA

The External Case Manager performs activities and duties related to preadmission evaluation and admission of patients to services along the rehabilitation continuum of care. Also responsible for relationship building for marketing and patient census building and coordination of care in collaboration with the acute care Case Managers/Social Workers/Discharge Planners to facilitate a smooth transition and most appropriate level of care. The External Case Manager works with appropriate funding sources to establish the most effective manner of managing the human, material and financial resources available for the patient's care.

- Upon hire: Washington Registered Nurse LicenseÂ
- 3 years of Clinical experience.
Preferred qualifications:Â
- Bachelor's degree in nursing.
- Upon hire: CRRN and/or CCM certificationsÂ
- Experience in case management; clinical nursing or utilization management.
- Experience in the rehabilitation of chronically or catastrophically ill or injured individual.
