Responsibilities

SUMMARY STATEMENT & PURPOSE

The Pharmacist in coordination with the Director of Pharmacy is responsible for the complete and efficient operation of the pharmacy within all legal and accreditation/certification requirements and applicable facility policies, procedures, codes and standards.  This includes pharmaceutical and professional functions relating to pharmaceutical care provided at this facility.

All employees are expected to be pleasant, respectful, and courteous in all interactions with patients, families, staff, and visitors of the Hospital, as well as being a positive representative of INBH at all times.

 

MINIMUM QUALIFICATIONS

  • Graduate from an accredited Pharmacy Program.
  • Currently licensed as a Pharmacist in Washington State.
  • Must possess the experience to oversee the provision of medication requirements for the patient populations treated in this facility, including knowledge of dosage for age-specific populations.
  • A working knowledge of Joint Commission Standards, OSHA regulations, and state and federal laws and regulatory requirements.

 

PREFERRED QUALIFICATIONS

  • A minimum of one (1) hospital pharmaceutical experience

Qualifications

SPECIFIC PERFORMANCE RESPONSIBILITIES

All functions are essential functions unless otherwise noted.

The job functions of this position are not limited to the duties listed below.

 

  1. Supervise the activities of the pharmacy technician and check all work conducted prior to delivery of medications to the communities.
  2. Assist the Director of Pharmacy to perform pharmaceutical distribution, chemicals and pharmaceutical preparations, safely, accurately, and according to state and federal regulations.
  3. Procure medications and dispense medications to the Automatic Dispensing machines and or medication cabinets/carts
  4. Review/audit Automatic Dispensing machine reports and understand process to troubleshoot solutions.
  5. Demonstrate appropriate judgment in communicating with physicians/licensed practitioners, nurses and other facility staff.
  6. Review all patient medication orders and document pertinent information in a timely manner in the pharmacy software program within interventions.
  7. Monitor patient profile for drug interaction, adverse drug reactions, and all other aspects of appropriate drug therapy.
  8. Effectively monitor the formulary to enhance the effectiveness of treatment, and achieve cost controls.
  9. Appropriately fill and label all drug containers when filled from pharmacy vs dispensed from Omnicell.
  10. Dispense all narcotic drugs and maintain appropriate inventories and State/Federal regulations.
  11. Properly dispose of hazardous pharmaceutical waste when applicable
  12. Ensure proper distribution, procurement, and clinical services functions within the pharmacy.
  13. Inspect all pharmaceutical supplies and services provided by the pharmacy.
  14. Provide counseling and consultation concerning medication to patients, families and facility staff, including providing educational materials, outpatient services, and samples as required.
  15. Provide Long Acting Injectable medications in house or through outside pharmacy. Administer and/or educate medication RN LAI when applicable.
  16. Consult (verbally or in writing) when requested or when appropriate.
