Position Summary The role of APRN/PA is responsible for providing direct patient care, assisting physicians with procedures, and assisting with other duties, as needed. Essential Position Responsibilities Perform age-appropriate history and physicals for PACE population. Orders, interprets diagnostic tests relative to patient's age-specific need and co-morbidities. Implement interventions to support patient stability. Actively participates in Interdisciplinary Team regarding care. Completes assessments as per PACE Regulations Documents services in the Electronic Medical Record in a timely manner. Serves rotation of On-Call status. Provides direct patient care, diagnosing, and treating health problems and coordinating health care for PACE population in accordance with State and Federal rules and regulations. Participates in the overall development of Plans of Care for PACE population. Skin and Wound Care Prescribes appropriate treatment modalities, performs medication reconciliation on PACE population. Serves as a Liaison with specialists in the community. Qualifications Education: Must be a graduate of an accredited Family Nurse Practitioner or Physician Assistant program Licensure and Certifications: Nurse Practitioner or Physician Assistant License and CPR, required. National certification, required. Experience: Minimum 3 years’ experience in a previous geriatric clinical nursing role, required. Work Environment: This position will spend 80% of time standing and/or walking while pushing, pulling, lifting, and/or carrying up to 50lbs.
