Location:

  • 100 Wasson Ave Springfield, MA

Schedule:

  • Full-time, 40 hours
  • Monday - Friday (8am-430pm)
  • On-Call rotation

Under the direction of nursing and physician leadership, the Transplant Nurse is responsible for coordinating and delivering comprehensive care, education, and support to transplant candidates, living donors, waitlisted patients, and post-transplant recipients in both inpatient and outpatient settings.

This role partners with a multidisciplinary team to provide patient-centered care, including assessment, care planning, case management, and education. Responsibilities include facilitating inpatient care coordination, discharge planning, and transitions of care, as well as providing telephone triage and longitudinal patient management throughout the transplant or donation journey.

Education is a core component of this role, including patient, family, donor, staff, and community outreach education. The coordinator also contributes to program development, quality improvement, and regulatory compliance.

SUMMARY:

  • Coordinate evaluation, work-up, and listing processes for transplant candidates and living donors; manage longitudinal follow-up for post-transplant patients.
  • Utilize the nursing process to assess, plan, implement, and evaluate individualized patient care across the continuum.
  • Collaborate with physician leaders and interdisciplinary teams to design and improve care delivery systems.
  • Demonstrate advanced knowledge of kidney function, chronic kidney disease (CKD), and end-stage renal disease (ESRD) treatment options.
  • Facilitate multidisciplinary rounds, patient care conferences, and transplant selection committee presentations.
  • Coordinate inpatient admissions, transitions of care, and discharge planning to optimize patient flow and outcomes.
  • Provide comprehensive patient, family, and donor education, adapting teaching strategies to individual needs.
  • Deliver telephone triage and clinical guidance to pre- and post-transplant patients and living donors.
  • Communicate effectively with referring providers, dialysis centers, and external partners to ensure continuity of care.
  • Participate in Quality Assurance and Performance Improvement (QAPI) initiatives and support regulatory compliance (e.g., CMS, OPTN).
  • Serve as a clinical resource and subject matter expert for transplant-related or living donation related care within the organization and community.
  • Contribute to staff and provider education, including outreach to dialysis centers and nephrology practices.
  • Maintain knowledge of tissue typing, transplant processes, and evolving clinical practices.
  • Ensure compliance with confidentiality standards and ethical care practices for all patients and donors.
  • Participate in committees and community outreach activities, including organ donation awareness initiatives.

Required Experience: 

  • Minimum of 3 years of nursing experience
  • Strong clinical assessment, critical thinking, and care coordination skills

Preferred Experience: 

  • 5+ years of relevant nursing experience
  • Experience in renal, dialysis, and/or transplant care
  • BSN preferred
  • Bilingual skills

Note: The compensation range(s) in the table below represent the base salaries for all positions at a given grade across the health system. Typically, a new hire can expect a starting salary somewhere in the lower part of the range. Actual salaries may vary by position and will be determined based on the candidate's relevant experience. No employee will be paid below the minimum of the range. Pay ranges are listed as hourly for non-exempt employees and based on assumed full time commitment for exempt employees.
