Sage Group HealthCare Staffing is seeking a LPN Home Health for a 13-week travel/contract position in Fairfax, VA. Accepting both local and traveler candidates.

Pay Rate: Range $1680 to $1720 Weekly Gross pay based on a 40-hour work week (Includes Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Mileage Reimbursement: $0.50/mile for distance covered between patient home appointments

Unit: Home Health

Shift: 8 Hours; Days, 8.00a - 5.00p

Schedule: 40 hours/week guaranteed, 5 shifts per week, Mon-Fri, Must meet assigned productivity points per week

Position Requirements:

LPN experience: 2 years - Required
Recent/Current Home Health LPN Experience: 1 year within the past 3 years – Required
Home Care Home Base (HCHB) charting experience: Recent/current - Required
Must be Covid Vaccination with booster - No Declinations, booster can be obtained upon offer. Exemptions allowed only if not initially vaccinated. Must be signed by provider/religious leader as applicable.

Education, License and Certification Requirements:

LPN License for State of VA - Required
BLS Certification from AHA - Required

Job Description:

Under general supervision, provide and coordinate care for assigned Home Health patients
Empower patients and families to reach healthcare goals by educating them on disease management.
Under general supervision, provide Home Health nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
Under general supervision, ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
Under general supervision, ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
Notify physicians and/or supervising clinician of significant changes in status.
Document and complete charting
Territories: Fairfax & Prince William County area and 50 miles radius
