The Hospice RN, Case Manager plans, organizes and directs hospice care services and is experienced in nursing, with emphasis on community health education/experience. The RN, Case Manager builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

This is a full-time position and will supporting the Covenant Living of Holmstad location (Batavia) and the surrounding locations.

In this role, the RN Case Manager will:

  • Completes an initial assessment of patient and family to determine hospice care needs.
  • Provides a complete physical assessment and history of current and previous illness(es).
  • Regularly re-evaluates patient nursing needs.
  • Initiates and oversees the plan of care and makes necessary revisions as patient status and needs change. Communicates updates as necessary.
  • Uses health assessment data to determine nursing diagnosis.
  • Develops a care plan, which establishes goals based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
  • Counsels the patient and family in meeting nursing and related needs.
  • Provides health care instructions to the patient as appropriate per assessment and plan of care.
  • Acts as member of Interdisciplinary Team and participates in IDT meetings and case conferences.

EDUCATION AND WORK EXPERIENCE:

  • Required Degree: Associate’s Degree, Nursing
  • Preferred Degree: Bachelor’s Degree, Nursing
  • Preferred Certificate(s): RN License and CPR, if required by state
  • Experience: 1+ Years of hospice care experience preferred

KNOWLEDGE, SKILLS, AND ABILITY:

  • Ability to communicate effectively with residents and healthcare professionals.​
  • Knowledge of cultural, social, and economic factors that influence patient care and healthcare access.​
  • Ability to report and document financial information as needed.​
  • Knowledge of privacy regulations and maintaining patient confidentiality.

PHYSICAL REQUIREMENTS:

  • Ability to stand, sit, talk, reach, stoop, kneel, and use hands and fingers to operate a computer, telephone, and keyboard.
  • Ability to lift, carry, push, and/or pull light to moderate amounts of weight up to 100 lbs.
  • Ability to operate various equipment base on department.
  • The ability to hear, understand, and distinguish speech and/or other sounds (e.g., machinery alarms, medical codes or alarms. Four hearing requirements listed: In person speech, Telephone, Other remote speech and other sounds.
  • Clarity of Vision:
    • For near visual acuity Clarity of vision at approximately 20 inches or less (i.e., working with small objects or reading small print), including use of computers.
    • For far visual acuity, Clarity of vision at 20 feet or more. This is not just the ability to see a person or object, but the ability to recognize features as well.
    • For peripheral vision, observing an area that can be seen up and down or to right or left while eyes are fixed on a given point.

Compensation Pay Range:

$35.09 - $49.33 per hour

Reasonable Pay Estimate

A reasonable estimate of the pay range for this position is $35.09 - $49.33 per hour. There are numerous factors taken into consideration in determining the actual offered rate of pay, including but not limited to: job-related qualifications, experience, skills, education, geographic location, and consideration of internal and external equity.
