Job Summary
The Director of Human Services & Care Coordination provides leadership and oversight for Dorchester Seniors, Inc.’s (DSI) human services, case management, care coordination, resource navigation, and rural outreach functions throughout Dorchester County.
The Director leads DSI’s Intervention & Stabilization Services, connecting members and community residents with resources, support, and services that promote safety, stability, independence, and quality of life. This includes overseeing services for individuals with increasingly complex needs and ensuring they are appropriately connected to DSI programs, healthcare providers, government programs, community resources, and other support systems.
This is a hands-on leadership position requiring a strong combination of human services knowledge, staff leadership, program management, community relationship building, compliance oversight, and organizational discipline. The Director will manage established Intervention services while helping DSI develop and strengthen Stabilization services for individuals requiring more intensive support.
The ideal candidate is a collaborative, compassionate, and highly organized leader who can supervise human services professionals, establish effective systems and protocols, develop strong community partnerships, monitor program performance, and expand access to services—particularly for older adults and residents living in rural and underserved areas of Dorchester County.
The primary office for this position is located at DSI’s St. George Center. This is not primarily an office-based position; regular field presence throughout Dorchester County is expected. Travel will include DSI’s Summerville Center, rural communities, healthcare and community partner locations, meetings, and outreach activities. A DSI vehicle will be provided for authorized business travel.
This position does not independently diagnose, provide clinical treatment, or make clinical determinations outside the individual’s professional scope of practice. Clinical, member-safety, compliance, and scope-of-practice concerns must be appropriately escalated to the CEO/Executive Director and/or qualified professionals.
Essential Duties & Responsibilities
The responsibilities listed below represent the primary functions of this position. Responsibilities may evolve as DSI’s programs and services continue to grow.
Human Services & Care Coordination Leadership
- Provide strategic and operational leadership for DSI’s Intervention & Stabilization Services across Dorchester County.
- Oversee case management, care coordination, resource navigation, rural outreach, social-service support, and related human services.
- Ensure services are responsive to member and community needs while supporting DSI’s mission and organizational priorities.
- Establish clear standards and protocols for referrals, intake, case coordination, documentation, follow-up, escalation, and warm handoffs.
- Ensure individuals with complex needs are appropriately connected to internal DSI services and external resources.
- Establish clear procedures for receiving referrals from DSI’s Prevention tier when member needs escalate.
- Develop systems that allow individuals to move appropriately between DSI service tiers as their needs change.
- Identify service gaps, emerging community needs, and opportunities to strengthen DSI’s continuum of support.
- Help develop, implement, and strengthen DSI’s Stabilization services for individuals requiring more intensive assistance.
- Establish workflows, procedures, documentation standards, and performance expectations necessary to support new and existing services.
- Ensure care plans, protocols, professional boundaries, and scope-of-practice requirements are documented and consistently followed.
- Immediately escalate significant compliance concerns, member-safety incidents, mandatory-reporting concerns, or scope-of-practice questions to the CEO/Executive Director.
Staff Leadership & Supervision
- Directly supervise the Case Manager and Rural Services Manager, as well as other Intervention & Stabilization staff assigned to the department as services develop.
- Establish clear performance expectations, responsibilities, priorities, and measurable goals.
- Conduct regular supervision, coaching, and performance-management meetings.
- Provide timely feedback and professional-development support.
- Monitor staff workloads and caseloads to promote appropriate capacity and quality service delivery.
- Ensure case records and other required documentation are accurate, complete, timely, and maintained according to established standards.
- Ensure clear protocols are followed for intake, referrals, case assignment, follow-up, escalation, and case closure.
- Address performance, documentation, service-delivery, and compliance concerns promptly.
- Promote collaboration between DSI departments so members receive coordinated services.
- Build a departmental culture centered on accountability, compassion, professionalism, confidentiality, collaboration, and service excellence.
Case Management & Resource Navigation
- Provide administrative and programmatic oversight of DSI’s case-management and care-coordination functions.
- Ensure members with complex social, economic, functional, or community-support needs receive appropriate professional assistance and referrals.
- Monitor caseload activity and identify trends affecting members and service delivery.
- Ensure appropriate warm handoffs occur between DSI programs and external partners.
- Support staff in identifying and maintaining knowledge of available community resources.
- Ensure mandatory-reporting responsibilities are understood and fulfilled.
- Maintain clear professional and organizational boundaries regarding services DSI can and cannot provide.
- Ensure situations requiring specialized expertise are appropriately referred or escalated.
Rural Outreach & Access
- Lead DSI’s rural outreach strategy as an organizational priority.
- Direct the Rural Services Manager’s work to identify, engage, and connect older adults and community residents in rural and underserved areas with needed services.
- Develop strategies to increase awareness of DSI programs and improve service access throughout western and upper Dorchester County.
- Build relationships with faith communities, healthcare providers, community organizations, government entities, local leaders, and other organizations serving rural communities.
- Identify opportunities for community outreach, education, referral partnerships, satellite services, and other methods of increasing DSI’s reach.
- Monitor rural outreach activity and outcomes to determine whether DSI is effectively reaching residents who may otherwise experience barriers to services.
Community Partnerships & External Relationships
- Develop and maintain a strong network of community partners supporting DSI’s human services and care-coordination work.
- Build productive relationships with nonprofit organizations, government agencies, healthcare systems, hospitals, clinics, home-health agencies, faith communities, social-service providers, and other community resources.
- Assist with developing healthcare relationships that can serve as referral sources and collaborative partners for DSI’s current and emerging services.
- Maintain awareness of available community resources and changes that may affect DSI members.
- Promote effective two-way referral relationships with partner organizations.
- Represent DSI at community coalitions, interagency meetings, collaborative planning groups, and other meetings as requested by the CEO/Executive Director.
- Ensure DSI’s Intervention & Stabilization Services are known and accessible to populations most likely to need them.
- Represent DSI professionally in interactions with members, community partners, healthcare organizations, government agencies, regulators, funders, and the public.
Program Development & Continuous Improvement
- Assist the CEO/Executive Director with developing and implementing new human services and community-based programs.
- Help design workflows, staffing structures, referral processes, procedures, documentation standards, and performance measures for new services.
- Evaluate existing processes and recommend improvements to strengthen service quality, efficiency, accountability, and outcomes.
- Use program data and community feedback to identify trends and opportunities for improvement.
- Support responsible program growth by ensuring new initiatives have appropriate systems, staffing, documentation, partnerships, and oversight.
- Help build sustainable systems that can grow with DSI rather than relying on informal or person-dependent processes.
Compliance, Confidentiality & Member Safety
- Ensure departmental operations comply with applicable DSI policies, procedures, contracts, funding requirements, confidentiality standards, mandatory-reporting obligations, and ethical expectations.
- Ensure sensitive member information is appropriately protected.
- Monitor staff compliance with documentation and confidentiality requirements.
- Ensure departmental procedures clearly establish professional responsibilities and scope-of-practice boundaries.
- Ensure employees understand when situations must be referred to healthcare professionals, protective services, emergency services, or other qualified professionals.
- Immediately escalate significant compliance concerns, member-safety incidents, or scope-of-practice questions to the CEO/Executive Director.
- Never independently make clinical judgments on behalf of DSI unless appropriately qualified and specifically authorized to do so.

Reporting, Data & Performance
- Prepare regular reports for the CEO/Executive Director regarding Intervention & Stabilization activity.
- Monitor and report on:
- Caseload and case-management activity

- Rural outreach
- Community-partner engagement
- Service utilization
- Member needs and trends
- Program outcomes

- Ensure all required data collection and reporting are accurate, complete, and submitted on time.
- Use data to evaluate program effectiveness and inform operational decisions.
- Identify performance or compliance concerns and develop corrective strategies.
- Contribute to grant applications, funder reports, monitoring activities, and Board presentations as requested by the CEO/Executive Director.
Budget & Resource Management
- Assist with developing and managing the departmental budget.
- Monitor departmental expenditures and use organizational resources responsibly.
- Identify staffing, technology, equipment, partnership, and other resource needs associated with service delivery and program growth.
- Assist with grant-funded programs and funding opportunities related to human services, care coordination, rural outreach, aging services, and community-based services.
- Work collaboratively with DSI leadership to ensure programs have appropriate resources and internal controls.
Organizational Leadership
- Serve as a member of DSI’s director-level leadership team.
- Participate actively in leadership meetings and organizational planning.
- Contribute to DSI’s strategic planning and implementation.
- Collaborate across departments to improve the member experience and organizational effectiveness.
- Help identify emerging needs among older adults and the broader community.
- Model DSI’s organizational values and professional standards.
- Support a culture of accountability, collaboration, continuous improvement, and service excellence.
- Attend Board of Directors meetings as directed by the CEO/Executive Director.
- Perform other duties consistent with the position as assigned.
Required Qualifications
- Bachelor’s degree in social work, human services, public health, healthcare administration, nonprofit management, psychology, sociology, gerontology, or a related field.
- Minimum of five years of progressively responsible experience in human services, healthcare, aging services, case management, care coordination, community services, outreach, or a related field.
- At least two years of supervisory, management, or program-leadership experience.
- Demonstrated ability to supervise employees, establish expectations, coach staff, and manage performance.
- Experience managing programs, projects, services, or community-based initiatives.
- Strong organizational and project-management skills.
- Ability to manage active programs while simultaneously developing new services, systems, or processes.
- Ability to manage or assist with management of a departmental budget.
- Excellent written, verbal, interpersonal, and relationship-building skills.
- Ability to work effectively with members, families, employees, healthcare professionals, community partners, government agencies, and other stakeholders.
- Ability to handle confidential and sensitive member information appropriately.
- Ability to understand and maintain professional and scope-of-practice boundaries.
- Ability to analyze program information, monitor performance measures, and prepare reports.
- Proficiency in Microsoft Office and/or Google Workspace; experience with case-management or care-coordination software preferred.
- Valid South Carolina driver’s license with no more than six points against the individual’s driving record.
- Ability to meet DSI’s requirements for authorization to operate a DSI vehicle.
- Ability and willingness to travel regularly throughout Dorchester County using a DSI-provided vehicle.
DSI’s current Vehicle and Driver Policy requires annual driving-record review for employees operating DSI vehicles and prohibits an employee from operating an agency vehicle with more than six points against their driving record.Â

Preferred Qualifications
- Master’s degree in social work, human services, public health, healthcare administration, nonprofit management, gerontology, or a related field.
- Experience working with older adults.
- Experience supervising case managers, care coordinators, social-service professionals, community-health workers, or outreach staff.
- Experience with CLTC, Medicaid waiver programs, home and community-based services (HCBS), or related programs.
- Knowledge of the human-services, healthcare, aging-services, and community-resource landscape in Dorchester County or a comparable South Carolina community.
- Experience working with hospitals, healthcare systems, home-health organizations, government agencies, or community-based providers.
- Rural outreach experience.
- Experience with grant-funded programs and associated documentation or reporting.
- Experience developing a new program, department, service line, or community initiative.
- Experience with case-management, care-coordination, or client-management software.
Clinical or social-work licensure is not required for this position unless subsequently required for a specific service or responsibility assigned to the role.
Work Environment & Travel
This is an on-site and community-based leadership position and is not a work-from-home position. Temporary, short-term exceptions may be approved under special circumstances by the CEO/Executive Director.
The Director is expected to maintain a regular field presence and will not work primarily from an office.
The position requires:
- Primary office presence at DSI’s St. George Center.
- Regular presence at DSI’s Summerville Center.
- Travel throughout Dorchester County.
- Visits to rural communities and community partner locations.
- Attendance at healthcare, government, nonprofit, coalition, and other community meetings.
- Participation in outreach activities and community events as necessary.
- Occasional schedule flexibility based on organizational and community needs.
A DSI vehicle will be provided for authorized business travel associated with this position. The employee is not expected to routinely use a personal vehicle to perform DSI field travel.
Employees operating DSI vehicles are required to follow DSI’s Vehicle and Driver Policy, including vehicle safety, documentation, accident-reporting, and driving-record requirements.Â

- Regularly sit, stand, walk, and reach.
- Travel by DSI vehicle throughout Dorchester County.
- Enter and exit an agency vehicle throughout the workday as field responsibilities require.

- Perform duties requiring appropriate manual dexterity and auditory and visual abilities.
- Lift and/or move items weighing up to approximately 30 pounds.

Compensation & Benefits
Salary: $62,400–$68,640 annually
