Additional Information About the Role

BJC is looking for a Care Management Specialist! This position would be working within pharmacy and collaborating with the chronic disease management team. This would be working on referral management, scheduling, communicating with providers, specialists, and patients. Good communication is key for this position. Epic experience is also a plus. 

 

Candidate will need to have Pharmacy Tech license for this position. 

 

Monday-Friday

8am-4:30pm

Preferred Qualifications

Role Purpose

Serves as the primary resource between the care management team, Providers and other patient care entities on projects that impact the care management of patients in the ACO (Accountable Care Organization). Responsible for collecting, developing and monitoring project documentation to implement and sustain patient-centered care activities.

 

Responsibilities

  • Collaborates with directors, managers and other disciplines to plan and implement projects related to patient care area.
  • Assembles project plans and teamwork assignments, directing and monitoring work efforts on a daily basis, identifying resource needs, performing quality review, and escalating functional, quality, timeline issues appropriately.
  • Ensures overall coordination, status reporting and stability of projects or operations-oriented work efforts.
  • Establishes methods to monitor the availability of reportable data and assists with planning and implementing new projects.
  • Works directly with leadership on internal business development activities, integration of business and quality goals and objectives, scope of services, and compliance planning.
