SUMMARY

Responsible for planning, directing, and evaluating all aspects of the hospice volunteer program. This role focuses on recruiting, onboarding, and retaining volunteer workers who contribute their services to enhance the quality of life for terminal patients and their families. This role develops and coordinates delivery of bereavement services to family members and/or caregivers for up to thirteen months following the death of a hospice patient. As a Licensed Social Worker, this role supports the hospice clinical team in functioning as one of the four core Hospice team members of the Hospice Inter-disciplinary Group (IDG) when needed. 


PRIMARY RESPONSIBILITIES

  1. Provides a complete range of Social Work Services to hospice patients including but not limited to planning, treatment and consultation. *
  2. Provides assessment of the psychosocial and spiritual needs of the patient and family. Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, high risks for suicide, neglect or abuse, bereavement rating, volunteer needs and plans interventions based on evaluation findings. *
  3. Assists the patient and family in developing and implementing an appropriate plan of care, and updates as needed. Counsel patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties and provides crisis intervention as needed. *
  4. Provide bereavement services to family members and/or caregivers; develop Individualized Bereavement Care Plan and follow-up as needed and as stipulated by policy and regulation. *
    1. Provide short-term individual counseling as warranted and facilitate referrals for clients as deemed appropriate.

    2. Determine scope and length of services required on a case-by-case basis.

    3. Facilitate referrals as necessary, utilizing Community contacts and external agencies as applicable.

  5. Coordinate valuations, surveys, and assignment of Bereavement Risk Ratings to ensure provision of services as necessary. *

  6. Coordinate education and outreach programs, facilitate grief support groups, and coordinate hospice memorials. *

  7. Recruitment and onboarding of hospice and bereavement volunteers. *

    1. Source volunteer candidates by traditional and creative means of networking and recruiting within the community.

    2. Screen and interview candidates.

    3. Deliver standardized hospice orientation and end-of-life care education applicable to the role of the volunteer.

  8. Patient Matching and Scheduling of volunteers. *

    2. Match volunteer skills and availability with geographic location, personality traits, patient needs, and specific requests.

    3. Coordinate regular companionship, vigil, or respite visits.

  9. Supervision of hospice and bereavement volunteers and maintaining regulatory compliance. *

    1. Track volunteer hours and maintain clinical record documentation.

    2. Ensure documentation of services provided and patient reactions.

    3. Ensure adherence to agency and regulatory standards.

    4. Provide work-related feedback for volunteers. Follow up on performance-related issues and corrective actions as required.

    5. Provide ongoing support, mentorship, and recognition for volunteers.

    6. Tracking metrics to maintain the mandatory federal 5% cost-savings threshold.

  10. Serve as backup coverage for the medical social worker – Hospice when needed. *

  11. Participate in Quality Assessment and Performance Improvement (QAPI) as directed. *

  12. Support various other Home care and Hospice responsibilities as needed. *

JOB SPECIFICATIONS

Education, Experience and Credentials

  • Bachelor’s degree in social work required.
  • Must possess Licensed Master of Social Work (LMSM).
  • CPR certification required.
  • One or more years’ experience in hospice or home care is preferred.
  • One or more years’ experience in supervision preferred.

  • Understand and maintain confidential nature of organization and client information in accordance with HIPAA regulations and corporate compliance standards, including appropriate access of data housed within the Information Technology infrastructure. 
  • Possess effective problem-solving skills.
  • Strong computer skills, including Microsoft Word, Excel, Outlook, Internet, and other applications. Must be skilled in database management and record keeping.
  • Able to display high levels of customer service, responding promptly and thoroughly to the inquiries and needs of individuals both internal and external to the organization in accordance with our PROUD standards.
  • Able to understand and follow facility and department safety and emergency procedures.


PHYSICAL REQUIREMENTS NECESSARY TO PERFORM THIS JOB

  • Perform light work, with occasional or negligible lifting of objects weighing up to 20 pounds.
  • Communicate clearly and effectively both orally and in writing to all levels of people inside and outside the organization, including possessing good English-speaking skills, fluency and understandability.
  • Able to effectively use office equipment to review and create information.
  • Work at a desk for extended periods of time and perform long hours of work at a computer. 
  • Frequently stand and walk to move around office/building repeatedly throughout the day.
  • Able to work a flexible work schedule, including overtime, nights and weekends as required including attending events/meetings as needed.
  • Able to obtain reliable transportation.


TOOLS AND EQUIPMENT USED

  • Must be able to use telephone, calculator, copy machines, computers, printers and other office equipment.
  • Provide own means of transportation around the Twin Cities metro area to frequently transport self between events, meetings, etc. must possess current driver’s license.


WORK ENVIRONMENT

The work environment is indoors. The incumbent must be flexible about switching positions quickly in order to respond to the needs of patients, in responding to emergencies.
