This position works closely with MCIL's Transitional Services team to move individuals on waivered services from their current living situation into a home of their own, sometimes for the very first time. Our team works collaboratively with other service providers and individuals to ensure they have necessary furniture and household items as they make a major life transition. Duties include but are not limited to:

  1. Coordinating Transitional Services Referrals to ensure individuals are moved from licensed facilities or other dwellings to community-based living situations with appropriate furniture, household items, and moving services.
  2. Coordinating outreach to community service agencies to achieve consumers' goals and meet service needs.*
  3. Completing documentation, data entry, and case noting related to consumer contact including emails, phone calls, meetings etc. in company databases
  4. Placing online orders for household and furniture items, tracking orders, providing updates to consumer's teams, and troubleshooting missing items as needed
  5. Picking up and delivering orders at local stores and moving companies - use of own vehicle, proof of car  insurance and valid driver's license are required for the position 
  6. Provide exceptional customer service to case managers and consumers
