RVNAhealth is seeking a compassionate, experienced Licensed Clinical Social Worker (LCSW) to join our Home Health team. This field-based position provides psychosocial support, counseling, care coordination, and advocacy to patients and families receiving skilled home health services in their homes or facilities throughout Southern Fairfield County in Connecticut including Norwalk, Westport, Darien, Stamford, Greenwich, and surrounding communities.
As an integral member of our interdisciplinary Home Health team, you will help patients navigate the emotional, social, and practical challenges that accompany illness, promoting dignity, independence, and quality of life while supporting safe care in the home.
Full-Time | 30 Hours per Week
Schedule: Monday - Friday; Or flexibility offered for a 4-day work week
Position Summary
The Home Health Clinical Social Worker provides comprehensive psychosocial assessments, counseling, crisis intervention, community resource coordination, and ongoing support for patients and their families. Working under the physician-approved plan of care and in collaboration with nurses, therapists, physicians, and other healthcare professionals, the LCSW develops individualized interventions that address the social, emotional, financial, and environmental factors affecting patient health and well-being.

Key Responsibilities
- Conduct comprehensive psychosocial assessments and ongoing reassessments of patients and families.
- Provide short-term counseling, crisis intervention, and emotional support for patients and caregivers.
- Assist patients and families in understanding diagnoses, treatment recommendations, and available resources.
- Identify psychosocial, emotional, financial, and environmental barriers to care and develop appropriate interventions.
- Advocate for patients and families while promoting their goals, independence, and overall well-being.
- Coordinate referrals and maintain collaborative relationships with community agencies and support services.
- Facilitate discussions related to care planning, advance care planning, and goals of care when appropriate.
- Provide counseling and support related to serious illness, loss, grief, and end-of-life concerns.
- Collaborate with the interdisciplinary care team to ensure coordinated, patient-centered care.
- Complete timely, accurate documentation within the electronic medical record in accordance with agency standards.
- Participate in case conferences, staff meetings, peer consultations, and continuing education programs.
- Maintain compliance with Medicare, Medicaid, HIPAA, agency policies, infection prevention standards, and all regulatory requirements.

- Perform other duties as assigned.
Qualifications
- Master's Degree in Social Work from a program accredited by the Council on Social Work Education.
- Current Connecticut Licensed Clinical Social Worker (LCSW) license.
- Minimum of five years of post-master's social work experience.
- At least one year of healthcare experience required; home health experience preferred.
- Experience conducting psychosocial assessments, care planning, and family counseling.
- Strong communication, interpersonal, and crisis intervention skills.
- Ability to work independently while collaborating effectively within an interdisciplinary team.
- Demonstrated cultural sensitivity and ability to work with diverse patient populations.
- Proficiency with electronic medical records and point-of-care documentation.
- Valid driver's license, reliable transportation, and ability to travel throughout the assigned service area in all weather conditions.
