Stamford Health has an opportunity for a per diem Oncology Pharmacist! Reporting to the Supervisor of the Bennett Cancer Center pharmacy and Director of Pharmacy, this position is responsible for providing quality pharmaceutical care for the patients of the Bennett Cancer Center Department of Medical Oncology Hematology. 

Responsibilities: 

  1. Accurately prepares and dispenses medication orders per physician request according to established policies, procedures and protocols. 

  2. Demonstrates understanding of chemotherapy, biotherapy and immunotherapy regimens, potential drug-drug interactions and potential adverse events.

  3. Provides timely and accurate drug information to other health professionals and lay persons when required. Available to participate in education for physicians and counseling sessions for patients, educating on side effects, indications and directions for use. 

  4. Assures appropriateness of therapy and dose for all patient populations including age-specific care.

  5. Monitors patient therapy in accordance with departmental policies and procedures, protocols, and programs.   

  6. Carefully identifies, prevents, intervenes, and follows up on any drug -related problems,  including any actual or potential adverse drug events, working with physicians and nursing staff as appropriate.  Supervises pharmacy support personnel based on current laws and regulations. 

  7. Completes required continuous training and education requirements, including federal and state regulations such as USP 797-800, department specific requirements, and demonstrates knowledge and skills necessary to provide quality pharmaceutical care to all patients.

  8. Ensures safe and effective medication therapies for patients according to established medication regimens, orders, policies, procedures and protocols.

  9. Assures that therapeutic substitution policies are followed based on individual patient need.

  10. Participates in the quality improvement and medication use review activities of the department.

  11. Flexible with scheduling days/hours to meet needs of department. 

Qualifications/Requirements: 

  • B.S. Pharmacy, Pharm.D. preferred.  Minimum 1 year experience in admixture of chemotherapy, immunotherapy and biotherapies.

  • Licensed or eligible for license as a Registered Pharmacist in Connecticut. 

  • Previous experience in an institutional setting preferred.
