At One to One Health, we prioritize personalized, high-quality care that advances the health and well-being of the communities we serve. This role is central to delivering rehabilitative, patient-centered care as a Physical Therapist, evaluating and treating a range of musculoskeletal and functional conditions to restore mobility, reduce pain, and improve overall quality of life. Working collaboratively with providers and interdisciplinary teams, this position develops and implements individualized treatment plans that support recovery, injury prevention, and long-term wellness. The role requires strong clinical expertise, attention to detail, and a commitment to evidence-based practice in a dynamic, fast-paced environment. The ideal candidate is dependable, compassionate, and dedicated to fostering meaningful patient relationships while upholding the highest standards of clinical excellence and care. Key Responsibilities: Perform initial evaluations and develop individualized treatment plans Implement therapeutic interventions (exercise, manual therapy, modalities) Monitor progress and adjust plans based on clinical response Educate patients and caregivers on home exercise programs and injury prevention Maintain accurate, timely documentation in the EMR Coordinate care with physicians and other healthcare professionals Ensure compliance with clinical, regulatory, and safety standards Qualifications: Doctor of Physical Therapy (DPT) from an accredited program Active state PT license (or eligibility) CPR/BLS certification Strong clinical reasoning and communication skills Qualifications
