Responsibilities

The ideal candidate will advocate for resident well-being, support families through care transitions, and ensure compliance with all social service regulations and standards
This position plays a vital role in enhancing residents' quality of life through assessment, counseling, care planning, and coordination of services
Conduct psychosocial assessments and update care plans based on resident needs
Provide counseling and supportive services to residents and families
Assist with admission, discharge, and transfer processes, ensuring appropriate communication with physicians and families
Facilitate discharge planning, including coordination with community resources and services
Advocate for resident rights and provide education to staff on psychosocial issues
Document social services interventions and progress in resident records
Participate in interdisciplinary care conferences and survey preparation
Ensure compliance with federal, state, and facility regulations related to social services
Ability to move freely throughout the facility and interact with residents and families
Occasional lifting of up to 40 lbs and pushing wheelchairs as needed
Occasional evenings or weekends as needed
