Make an Impact Beyond the Pharmacy Counter

Noble Community Clinics is seeking a Pharmacist to join our integrated healthcare team. This position provides an opportunity to combine traditional pharmacy practice with direct patient care, medication management, clinical consultation, and population health initiatives.

As a Pharmacist at Noble, you will work closely with medical, dental, behavioral health, and other care team members to help patients achieve better health outcomes. You’ll play an important role in medication safety, patient education, regulatory compliance, and the continued development of pharmacy services within our organization.

What You’ll Do

Clinical Pharmacy & Patient Care

  • Provide direct patient care as part of an integrated primary care team.
  • Conduct comprehensive medication reviews and obtain medication histories.
  • Develop evidence-based, patient-centered medication recommendations.
  • Educate patients on medications, dosing, potential side effects, and adherence.
  • Review laboratory results and clinical data to support therapeutic decisions.
  • Administer and document vaccinations and injectable medications.
  • Identify medication-related concerns, drug interactions, contraindications, and adverse effects.
  • Review PDMP information prior to dispensing controlled substances.

Medication Management & Pharmacy Operations

  • Review and verify prescriptions for accuracy, appropriateness, and legal compliance.
  • Dispense medications in accordance with professional standards.
  • Maintain accurate patient medication records.
  • Oversee medication inventory, ordering, storage, and expiration monitoring.
  • Maintain appropriate controls and documentation for controlled substances.
  • Support day-to-day pharmacy operations and provide coverage as needed.

Compliance & Quality

  • Maintain compliance with federal and Wisconsin pharmacy laws and regulations.
  • Ensure adherence to HRSA 340B program requirements.
  • Support patient safety, risk management, and accreditation standards.
  • Participate in quality improvement and peer review activities.
  • Identify opportunities to improve pharmacy processes and patient outcomes.

Leadership & Collaboration

  • Collaborate with providers and care teams across medical, dental, behavioral health, and other disciplines.
  • Support the development and expansion of clinical pharmacy programs.
  • Supervise and support pharmacy technicians and intern pharmacists.
  • Precept and mentor pharmacy students.
  • Provide medication-related education and training to staff and healthcare team members.

What We’re Looking For

  • Doctor of Pharmacy (PharmD) from an ACPE-accredited College of Pharmacy.
  • Current Wisconsin Pharmacist license in good standing.
  • Basic Life Support (BLS) certification required.
  • Immunization certification preferred.
  • Experience in ambulatory care, community health, FQHC, or integrated care preferred.
  • Strong clinical judgment, critical thinking, and problem-solving skills.
  • Excellent communication and collaboration skills.
  • Strong attention to detail and commitment to medication safety.
  • Ability to work effectively both independently and as part of an interdisciplinary team.
  • Proficiency with EHR/EMR and pharmacy information systems.
  • Knowledge of pharmacology, therapeutics, drug interactions, dosage calculations, and evidence-based clinical guidelines.
