Palliative Nurse Practitioner:

Concordia Visiting Nurses is seeking a compassionate and experienced Palliative Care Nurse Practitioner to join our growing team! • Experienced in palliative care, hospice, home health, geriatrics, or chronic disease management

• Licensed and board certified as a Nurse Practitioner in Pennsylvania

What You’ll Do as a Nurse Practitioner:
• Provide comprehensive palliative care assessments and symptom management
• Support patients and families through goals-of-care discussions and advanced care planning
• Collaborate with physicians, nurses, social workers, and interdisciplinary team members
• Manage complex and chronic conditions with a focus on comfort and quality of life
• Educate patients, caregivers, and staff on palliative care services
