聽 聽 聽 聽 聽 聽 A Case Manager is responsible for providing a wide variety of services and supports to the individuals we serve in order to聽help them to maximize their quality of life. The position provides case management services to a group of consumers living in various community living arrangement programs or their own homes throughout the county. This includes initial intake interviews, appropriate referrals, liability information and completion of progress notes, consumer contacts as well as ongoing services such as coordination and monitoring.聽
聽 聽 聽 聽 聽 聽 A person in this position is expected to demonstrate the system of care practice model, which maximizes all resources around an individual or family. The system of care includes child and adult serving agencies and community partners that join with an individual or family to meet their goals.

Education and Experience
聽 聽 聽 聽A bachelor's degree which includes or is supplemented by successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences;
OR
聽 聽 聽 聽Two years of experience as a County Social Services Aide 3 and two years of college level course work which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences;
OR
聽 聽 聽 聽Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of experience as a County Social Services Aide 3 or in a similar position performing paraprofessional case management functions.
Physical Demands
聽 聽 聽 聽 聽 聽 While performing the responsibilities of the Case Manager, the employee is required to talk and hear. The employee is often required to sit and use their hands and fingers, to handle or feel and to manipulate keys on a keyboard. The employee may be required to stand, walk, reach with arms and hands, climb or balance, stoop, kneel, crouch or crawl and, enter and exit a vehicle. 聽The work may require constant mobility. The employee may need to work with arms above shoulder height, bend at the waist or work bent at the waist, lift or carry items or occasionally work with an uncommon level of noise. Vision abilities required by this job include close vision.
Work Environment聽
聽 聽 聽 聽 聽 聽聽Normal office environment. The noise level in the work environment is usually quiet to moderate. Position may require travel throughout Carbon, Monroe and Pike counties in addition to periodic travel to other locations in Pennsylvania. An agency vehicle is usually available during work hours.

Position Duties:聽
聽 聽 聽 聽聽Assess consumer needs and develop proposed services plans/life management plans as per departmental schedules.
聽 聽 聽 聽聽Assure implementation and coordination of the plan.
聽 聽 聽 聽聽Monitor consumer programs as per established case management standards.
聽 聽 聽 聽聽Maintain records, correspondence, service plans and authorizations for each consumer.
聽 聽 聽 聽聽Make necessary referrals and coordinate services as prescribed in the consumer proposed services plan/life management plan, e.g., core team, in home therapy, camp, residential services, day program services, etc.
聽 聽 聽 聽聽Have weekly supervisor conferences on specific cases on caseload.
聽 聽 聽 聽聽Prepare monthly supports coordination statistics.
聽 聽 聽 聽聽Attend staff conferences of other agencies as they relate to case management services.
聽 聽 聽 聽聽Assure close working relationship with other human services agencies as they relate to assigned caseload.
聽 聽 聽 聽聽Close cases no longer necessitating case management services.
聽 聽 聽 聽聽Other duties as assigned by Supervisor.
聽 聽 聽 聽聽Work will be supervised by Supervisor through regularly scheduled conferences and on an as needed basis.

Additional Duties:
聽 聽 聽 聽聽Performs related work as required.
聽 聽 聽 聽聽Use of personal vehicle may be required.
