Primary Social Worker -  Sturgeon Bay & Northern Door County


$5,000 Sign-On Bonus

We're looking for a dedicated Social Worker who is passionate about providing emotional support, advocacy, and guidance while making a lasting difference in the lives of others.

Schedule & Territory:

  • 30 hours per week (benefit eligible - full-time) 
  • Choose the schedule that works best for you: 
    • Three 10-hour days, or 
    • Four 7.5-hour days 
  • Territory includes Sturgeon Bay and Northern Door County, including Washington Island 
  • Interdisciplinary Team (IDT) meetings every other Thursday at 7:00 a.m. 
  • One holiday per year 
  • Shared on-call rotation: 
    • One weekend per month or every other month (based on team rotation) 
    • Friday: 5:00 p.m.–9:00 p.m. 
    • Saturday/Sunday: 8:00 a.m.–5:00 p.m. or 11:00 a.m.–8:00 p.m. 

What you'll do:


As a Primary Social Worker, you'll be an essential member of Unity's interdisciplinary hospice team, helping patients and families navigate the emotional, social, and practical aspects of end-of-life care.

You'll:

  • Complete psychosocial assessments and develop individualized care plans. 
  • Provide counseling, emotional support, education, and advocacy for patients and families. 
  • Connect patients with community resources and coordinate referrals. 
  • Participate in interdisciplinary care planning and collaborate with nurses, physicians, chaplains, aides, and volunteers. 
  • Support bereavement planning and assist with funeral planning when requested. 
  • Maintain timely and accurate documentation. 
  • Uphold the Social Work Code of Ethics while delivering compassionate, patient-centered care. 
  • Participate in a shared on-call rotation.

Requirements

  1. Bachelor’s in Social Work (BSW) from an institution accredited by the Council on Social Work Education; or bachelor’s degree in psychology, sociology, or other fields related to social work along with one year in a healthcare setting. Master’s of Social Work preferred.
  2. Certified or licensed as a social worker (CSW, CAPSW, CISW, or LCSW) by the State of Wisconsin required.
  3. Valid Wisconsin Driver’s License, reliable transportation, and proof of current automobile insurance coverage.
