Hospice RN Case Manager

**$10k Sign on Bonus**

Lake Prince At Home a part of Lake Prince Woods, CCRC, located in Suffolk has a CHAPS Certified Hospice and Home Health organization. We are seeking an experienced Hospice Nurse who is looking for an opportunity to grow within this new service line.

The Hospice RN Case Manager is accountable for the delivery of quality home care services to the program's patients in conjunction with physician's directions, company and industry standards, and insurance guidelines as assigned.

POSITION REQUIREMENTS:

Minimal Education: Graduate of accredited school of nursing.2. Bachelor’s degree of Science in Nursing is preferred. Experience as Hospice Nurse.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Follows established Standard of Nursing Practices and implement Facility policies and procedures. Knowledgeably and competently performs nursing skills. Identifies safety hazards and initiates corrective action immediately including assuring the safety of Residents under his/her supervision. Upon admission and on an ongoing basis, assesses the patient’s physical and emotional status, level of competency/knowledge, and safety. Identifies health and safety hazards. Develops an accurate comprehensive care plan based on each resident's assessment. Provides direct resident care and gives instruction to Nursing Assistants. Formulates and periodically updates care plan as necessary and as required by Regulations. Participates in interdisciplinary case conference. Records and reports changes in the patient’s condition to physician. Documents all orders in accordance with regulatory and facility requirements. Maintains current technical and clinical skills, certifications and licensing.
