Job DetailsLevel: ExperiencedJob Location: Destiny Springs Hospital Main - Surprise, AZ 85378Position Type: Full TimeEducation Level: Graduate DegreeSalary Range: $31.00 HourlyTravel Percentage: NoneJob Shift: Day Position Summary: a { text-decoration: none; color: #464feb; } tr th, tr td { border: 1px solid #e6e6e6; } tr th { background-color: #f5f5f5; } The Social Worker/Counselor is an active member of the patient's interdisciplinary treatment team and is responsible for ensuring that the medically related emotional and social needs of patients are met and maintained on an individual basis in accordance with organizational policies and procedures, as well as applicable federal, state, and local regulations and standards. Responsibilities include conducting initial psychosocial assessments, developing and updating treatment plans, facilitating psychoeducational and process groups (including Mindability: Roadmap to Peace of Mind), participating in treatment team meetings, coordinating care with outpatient providers and community resources, discharge planning, and maintaining accurate and timely documentation within the electronic medical record (EMR).   ESSENTIAL FUNCTIONS: Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.   •       Develops comprehensive social histories and psychosocial assessments that identify each patient’s strengths, needs, preferences, cultural considerations, and implications for the treatment plan. •       Orients patients and families to the facility, available services, service limitations, patient rights, and applicable expectations. •       Involves patients and families, as appropriate, in treatment planning by identifying individualized, measurable goals. •       Monitors patient progress toward treatment plan goals and updates the treatment team regarding identified needs, barriers, and progress. •       Attends scheduled treatment team meetings and communicates psychosocial needs, discharge barriers, and coordination-of-care updates for inclusion in the treatment plan. •       Completes timely, accurate, and compliant documentation in the electronic medical record in accordance with applicable regulations, facility policy, and established timelines. •       Uses therapeutic communication, problem-solving, and appropriate intervention strategies to help address concerns involving patients, families, and staff. •       Coordinates referrals and linkage to appropriate community resources, including outpatient treatment providers, transportation resources, and other identified support services. •       Provides crisis intervention, therapeutic support, and de-escalation assistance within the scope of the role and in accordance with facility training. •       Develops comprehensive discharge plans beginning upon admission in collaboration with patients, families, multidisciplinary staff, referral agencies, and outpatient service providers. •       Facilitates psychoeducational and process groups using approved curriculum, including stress management, coping skills, Mindability-Roadmap to Peace of Mind, and other assigned programming. •       Maintains visibility and accessibility on assigned units while demonstrating appropriate professional boundaries. •       Complies with all applicable regulations, hospital policies, work procedures, and instructions.   SUPERVISORY RESPONSIBILITY: This position has no supervisory responsibility. SKILLS/KNOWLEDGE:  •       Effective verbal and written communication skills. •       Ability to problem-solve, exercise sound judgment, and make appropriate decisions within the scope of the role. •       Ability to adhere to safety policies, procedures, infection control standards, and facility expectations. •       Ability to recognize and respond appropriately to the needs of various patient populations, including adolescents and adults. •       Knowledge of behavioral health practices, psychosocial assessment, treatment planning, discharge planning, and coordination of care. •       Ability to understand and comply with applicable government regulations, regulatory standards, confidentiality requirements, and hospital policies. •       Ability to initiate action, take personal responsibility, recommend improvements, and resolve problems within the scope of the position. •       Customer/client-focused with strong interpersonal and relationship-building skills. •       Ability to use therapeutic communication, crisis intervention, and de-escalation techniques when working with patients in distress. •       Ability to complete timely, accurate, and compliant documentation in the electronic medical record. •       Knowledge of emergency response procedures, including facility code procedures. COMPETENCIES:   •       Technical Capacity. •       Performance Management. •       Ethical Conduct. •       Communication Proficiency. •       Personal Effectiveness, Credibility, and Integrity.     WORK ENVIRONMENT: This position operates in a behavioral health clinical setting and routinely requires direct interaction with patients, families, clinical staff, and members of the multidisciplinary treatment team. The role involves frequent documentation, use of the electronic medical record, and operation of standard office equipment. The employee may be exposed to emotionally distressed patients, crisis situations, frequent interruptions, noise, and a fast-paced care environment. The role requires the ability to remain calm, professional, and clinically appropriate when responding to patients who may exhibit disruptive, escalated, aggressive, or acting-out behaviors. The employee is expected to use therapeutic communication, crisis intervention, and de-escalation techniques in accordance with facility policies and training.   PHYSICAL DEMANDS: This is largely a sedentary role; however, some standing, lifting, bending and stooping is required.    TRAVEL: No travel required.       QualificationsREQUIRED EDUCATION / EXPERIENCE: •       Master’s degree in Social Work, Counseling, Psychology, or a related behavioral health field. •       Licensure with the Arizona Board of Behavioral Health Examiners preferred. •       CPR, BLS, and CPI certification required prior to direct patient interaction. •       Minimum of two years of related behavioral health, social work, or clinical experience required.
