Job Summary:

As the Clinical Social Worker, you will provide proactive care management for high-risk patients and their families by addressing psychosocial issues that may affect medical care and health outcomes. 

You will interview patients, families and caregivers to assess personal and social adjustments and look for any needed interventions to mitigate risks. Once identified, you will deliver interventions and connect patients to appropriate resources. 

In this role, you will focus on supporting families in managing complex conditions hindering overall well-being.

ACCOUNTABILITIES 

  • Utilizes risk stratification algorithms and pursuit lists to identify high-risk pediatric patients who may benefit from care management services.  

  • Collaborates with the inter-professional team to provide care management support for patients at risk of poor outcomes due to behavioral health concerns or chronic conditions.  

  • Interviews pediatric patients, families, and caregivers to assess personal and social adjustment, identifying necessary interventions to mitigate risks.  

  • Delivers interventions addressing barriers posed by mental, behavioral, or substance abuse disorders, connecting patients to appropriate resources.  

  • Collaborates with the pediatric multi-disciplinary team to implement treatment plans, ensuring alignment and continuity of care.  

  • Prepares psychosocial assessment histories and develops individualized therapy plans, including referrals, based on patient needs and goals.  

  • Counsels pediatric patients and families to enhance understanding of personal or situational problems and provides crisis intervention when necessary. 

JOB REQUIREMENTS 

  • Education:Master’s Degree in Social Work 

  • Skills: Exceptional problem-solving, critical thinking, organizational, interpersonal, and written/verbal communication skills. Ability to work in self-directed environment with attention to detail, follow through, and has a high level of computer literacy. History of positive rapport with patients, families, physicians, and interdisciplinary team. 

  • License: Active LSW license

  • Years of Experience: Experience in a medical or other multi-disciplinary agency is desirable, and experience that matches the assigned practice specialty.
