Constellation Health Services is seeking a Registered Nurse to join our start-up and survival team (S&S) in New York. As we expand our footprint, presentation, articulation, and a strong skill set are crucial for introducing our brand. Members of this team, depending on location, are asked to be the backbone of the site. **This position is one of a road warrior and requires great organization and task management. This role would be responsible for a minimum of 2-3 admissions (starts of care) per day, 12-14 per week. ** **This position is heavy in performing SOC assessments and will provide intermittent coverage and clinical support in NYC, to include Patient Care Services, Referrals, and Case Load Coverage as needed. ** Constellation Health Services was founded to be an innovative leader in the post-acute continuum by the development of core teams that embody clinical excellence, uniqueness within their communities, and act with authentic intention. Responsibilities: Deliver compassionate and comprehensive nursing care to patients Provides skilled nursing care, including administration of medications and treatments as prescribed in the physician’s plan of care for patients in their home setting. Conduct thorough assessments to identify patient needs and develop individualized care plans. Manage medications and treatments as prescribed by the physician. Provide emotional support and guidance to patients and their families. Educate patients and families about their plan of care and disease trajectory Collaborate effectively with interdisciplinary team members and attending physicians to ensure coordinated care. Maintain accurate and up-to-date patient records. Ability to travel up to 100% of the time. QualificationsQualifications: Current and active RN license in the state of New York Bachelor’s degree in nursing (BSN) preferred. Minimum of 2 years of field RN home care experience. In-depth understanding of certified home health. Exceptional communication and interpersonal skills. Ability to work independently and as part of a team. Compassionate and empathetic approach to patient care. Basic knowledge of iOs operating system.
