Job DetailsJob Location: Main Office - Syracuse, NY 13208Position Type: Full TimeSalary Range: $22.50 - $23.00 HourlyArise is looking for an Outreach and Transitional Specialist Job Summary: The Outreach &Training Specialist for the Open Doors Transition Center Program assists individuals transitioning from institutions to community settings and serves the nine-county region of Central New York (Cayuga, Onondaga, Oswego, Madison, Oneida, Lewis, Herkimer, Jefferson, and St. Lawrence). The Outreach & Transition Specialist will have two main responsibilities: provide essential community outreach and provide person-centered planning for participants. QualificationsQualifications: Bachelor’s degree in human services or related field plus 2 years of experience - or- Associate degree with at least 4 years of related experience Experience coordinating and facilitating group and individual training programs Excellent customer service and communication skills Organized and self-motivated Ability to work well independently, as well as part of a team. Proficient with Microsoft Programs, Internet, and database operations. Understanding of Independent Living philosophy or willingness to learn about it Clean driver license and reliable transportation for travel in the nine-county region. Essential Functions: Provide in person trainings to nursing home staff on Section Q of the Minimum Data Set (MDS) and Open Doors. Participate in the development of trainings for on-site staff, annual trainings, NYAIL conference, regional trainings, webinars, and one-on-one trainings. Manage/maintain outreach materials and current contact information for Nursing Homes. Work with participants to develop and implement a Person-Centered Plan (PCP) collaborating with social workers, service coordinators, case managers, family members, guardians, advocates, etc. to secure qualified housing and coordinate home and community services prior to transition. Explain peer mentor program to potential participants and if appropriate provide necessary referral Collaborate with the supervisor to determine and resolve barriers to transition, including barriers related to housing, medical condition, or counseling need. Ensure all significant participant data and service provision details are current and accurate and that they are documented in a timely manner into applicable databases Other duties as assigned. Work location: Syracuse - Onondaga County Hours and days: Monday-Friday 8:00am -4:00pm Compensation: $22.50 -$23.00 per hour Non -Exempt 40 hours per week, including one-hour paid lunch
