- Full-time, permanent opportunity
- Schedule: Days; Mon-Fri
- Oversees case management operations, including care coordination, discharge planning, and utilization review
- Leads and develops case management staff to support quality outcomes, regulatory compliance, and patient satisfaction
- Collaborates with physicians, nursing leadership, and interdisciplinary teams to optimize patient throughput and resource utilization
- The Director of Case Management must have an LCSW or RN license
- Must have 3 or more years of hospital based case management experience
- Have or be willing to obtain CCM or ACM
- Bachelor's Degree or higher required
