Puget Sound Home Health and Hospice is seeking an experienced and caring full-time Hospice Registered Nurse Case Manager (RN) in Pierce County, Washington!

What You’ll Be Doing:
- Complete comprehensive in-home patient assessments
- Manage patient comfort, symptom control, and pain relief
- Develop and update individualized plans of care under physician orders
- Deliver skilled, compassionate nursing care in the home setting
- Document care accurately and promptly per clinical and legal standards
- Participate in quality improvement initiatives (QAPI)
- Collaborate with an interdisciplinary team to ensure holistic patient support
What We’re Looking For:
- Current RN license in WA (or compact state)
- BLS certification from AHA
- Valid driver’s license, auto insurance, and reliable transportation
- Strong critical thinking and independent problem-solving skills
- Hospice agency experience preferred but not required
