Job DetailsJob Location: Bridgeport, CT 06604Position Type: Full TimeSalary Range: $55.00 - $60.00 HourlyTravel Percentage: Up to 25%Constellation Hospice is seeking an energetic, dynamic Registered Nurse (RN) to manage the General Inpatient (GIP) hospice patients at select hospitals.  This position will be responsible for evaluating and completing hospice starts of care and revisits.  Beyond this primary role, this position may also work as a Hospital Liaison. The Liaison will be responsible for expanding business opportunities in the assigned Hospitals.  This person will manage the clinical care coordination and work with various positions within the hospital including case management, physicians, APRNs, palliative teams, social work, and hospital management.  The Liaison will also be responsible for speaking with patients and families related to end of life, completing consents, ordering appropriate medications, DME and medical supplies and ensure appropriate admission and hospital discharges to ensure a safe, efficient and successful transfer of care. Hours for this role are Monday through Friday, daytime shift Location: Milford and Bridgeport Hospitals   Responsibilities Complete hospice evaluations, starts of care and revisits within the hospital. Assesses clinical status, and eligibility for the identified benefit, to determine the appropriateness for Hospice admission based on Hospice Criteria. Discusses clinical goals of care and makes recommendations for appropriate service selection. Performs full clinical assessment and handles GIP admissions and visits. Clinical focus on ensuring the patient's eligibility, required consent forms, correct coding and billing documentation. Responsible for generating referrals for our hospice program by building relationships with social workers, case managers, physicians, APRNs, and key decision-makers. QualificationsQualifications: Active RN License Minimum of 1-3 years of clinical experience in Hospice and/or Palliative Care. Ability to assess patient clinical status and discuss care options for the state of illness. Previous hospice sales experience preferred. Bachelor's degree preferred Excellent planning, organization and presentation skills are critical. Proven ability to develop and implement a sales and marketing plan. Excellent oral/written, communication and interpersonal skills. Computer Skills and basic knowledge of iOS operating system. This position requires an individual who is highly motivated, results-driven, and able to develop and build strong, lasting relationships.  The ideal candidate will have established healthcare contacts and be able to readily network in the community.   #IND1
