Regular, Full-Time position | Schedule: Mon-Fri 8a-4:30p | Pay: Paygrade 12, Salaried. Pay will be dependent upon experience as a LCSW

Job Purpose or Objective(s): 

The Licensed Clinical Social Worker helps patients access healthcare, advocates for their wellbeing, and educates patients on available resources. You will address barriers that affect care and recovery to support safe and effective patient outcomes.

Primary Tasks:

  1. Assess social, emotional, behavioral, environmental, and financial factors affecting patient care and recovery.
  2. Develop and implement patient-centered care plans in collaboration with interdisciplinary healthcare teams.
  3. Coordinate discharge planning to ensure safe and effective transitions to home, rehabilitation facilities, skilled nursing facilities, hospice, or other levels of care.
  4. Identify barriers to discharge and facilitate appropriate solutions and resource referrals.
  5. Educate patients and families regarding available community resources, support services, insurance benefits, and healthcare options.
  6. Facilitate advance care planning, advanced directives, and end-of-life discussions when appropriate.
  7. Conduct risk assessments related to abuse, neglect, substance use, homelessness, and patient safety.
  8. Document assessments, interventions, and discharge plans in the electronic medical record in accordance with hospital policies and regulatory standards.
  9. Participate in interdisciplinary rounds, case conferences, and quality improvement initiatives while advocating for patient rights and access to needed services in compliance with ethical and legal standards.
  10. Perform other duties as may be assigned.

 

 

Required Education, Skills, and Experience: 

  • Master’s degree in social work 
  • Current and in good standing Licensed Clinical Social Worker (LCSW) License
  • Strong communication, organizational, and collaboration abilities
  • Proficiency with electronic medical records (EMR/EHR)
  • Experience with Microsoft Office
  • Valid driver's license
  • Knowledge of HIPAA and Joint Commission, and Choctaw Nation policies
  • Three [3] years of experience in social work or related field
