Big Bend Hospice is seeking a Referral Specialist to serve as the vital first connection between patients, families, healthcare providers, and our care teams. In this full-time role, you will help coordinate referrals for Hospice, Home Health, and Palliative Care services, ensuring patients receive timely, compassionate support. Schedule: Full-time, M-F, 1:00pm - 10:00pm with rotating weekends and holidays. What You'll Do As a Referral Specialist, you will play a key role in creating a seamless admission experience for patients and referral partners by: Responding promptly and professionally to incoming referral calls. Gathering and verifying patient, insurance, and referral information. Coordinating admission visits with our clinical team. Scheduling appointments and ensuring timely communication with patients, families, physicians, and referral sources. Creating and maintaining accurate patient records within the electronic medical record (EMR) system. Collaborating with Admissions Nurses, Social Workers, Finance, and Patient Care teams to facilitate smooth admissions and reimbursement processes. Ordering equipment and supplies as requested to support patient care needs. Communicating referral updates to internal teams and community partners. Converting referrals to active admissions and maintaining accurate data throughout the process. Strong customer service and interpersonal skills. Excellent organizational and time management abilities. Attention to detail and ability to manage multiple priorities. Strong problem-solving and critical-thinking skills. Professionalism, integrity, and discretion when handling confidential information. Proficiency with Microsoft Office applications. Experience working with electronic medical records (EMR) systems. Valid Florida Driver's License, reliable transportation, and current auto insurance. Education & Experience High School Diploma or GED required. Associate degree preferred. Minimum of two years of administrative or clerical experience required. Experience in a healthcare or medical office setting preferred. Knowledge of medical records and healthcare terminology preferred. Active Florida Licensed Practical Nurse (LPN) license is a plus. Physical Requirements Ability to sit and work at a computer for extended periods. Ability to lift up to 15 pounds occasionally. Ability to access and navigate various departments within organizational facilities.
