Join our interdisciplinary hospice team and help patients and families navigate one of life's most meaningful journeys. As a Hospice Social Worker, you'll provide counseling, advocacy, education, and care coordination that truly changes lives. What You'll Do As a member of our interdisciplinary team, you will: Complete comprehensive psychosocial assessments Provide emotional support, counseling, and crisis intervention for patients and families Assist with advance care planning and community resource referrals Help families navigate Medicaid applications, financial resources, and long-term care planning Collaborate closely with physicians, nurses, chaplains, volunteers, and other members of the hospice team Participate in interdisciplinary care conferences Advocate for patients and families while honoring their goals and wishes Provide services wherever patients call home—including private residences, assisted living facilities, nursing homes, hospitals, and inpatient hospice care SCHEDULE:  Monday through Friday 8 AM to 5 PM  On Call Rotation once every 7 weeks and holidays What We're Looking For:What We're Looking For Required Qualifications Master's Degree in Social Work (MSW  Valid Florida driver's license Reliable transportation and current auto insurance Preferred Qualifications Two years of healthcare, hospice, home health, oncology, palliative care, or related experience Florida Licensed Clinical Social Worker (LCSW) or license-eligible Knowledge of grief and bereavement counseling Experience working with interdisciplinary healthcare teams Strong communication, organizational, and counseling skills Why You'll Love Working Here We believe caring for our team is just as important as caring for our patients.
