About the Role:

Responsibilities:
- Act as the primary point of contact for patients, addressing inquiries and providing support.
- Coordinate patient appointments and follow-up consultations to ensure continuity of care.

- Build and maintain strong relationships with healthcare providers and community organizations.
- Document patient interactions and maintain accurate records in compliance with HIPAA regulations.
- Collaborate with internal teams to enhance patient care processes and service delivery.

- Stay updated on industry trends and best practices to continually improve patient experiences.
Requirements:
- Proven experience in patient care or healthcare liaison roles 3-5 years
- Strong communication and interpersonal skills to foster relationships.
- Knowledge of healthcare regulations and patient privacy standards.
- Ability to work independently and as part of a collaborative team.
- Compassionate demeanor with a genuine interest in patient welfare.
- Valid driver’s license and willingness to travel within the Northern Indiana and surrounding areas.
- Relevant certifications in healthcare or patient advocacy are a plus.
- Cardiology experience preferredÂ
