Physical Therapist - Full Time

Clinical Magnet is seeking a dedicated Physical Therapist to join a collaborative healthcare team supporting adults age 55 and older in the Tampa area. Position Overview
The Physical Therapist is responsible for evaluating participants and developing individualized treatment plans designed to support functional mobility and safety. Care may be provided at an acute, restorative, or maintenance level depending on participant needs. Treatment plans are incorporated into the participant’s overall plan of care and carried out in collaboration with physicians and the interdisciplinary care team.
Key Responsibilities
- Conduct physical therapy evaluations and determine rehabilitation needs for program participants
- Develop and implement individualized treatment plans based on physician orders and participant goals
- Provide therapeutic exercise, functional mobility training, and other appropriate treatment interventions
- Communicate participant progress and recommendations with the interdisciplinary care team
- Educate participants, caregivers, and family members on safety, positioning, and mobility strategies
- Provide guidance on the proper use and care of durable medical equipment
- Delegate appropriate treatment activities to Physical Therapist Assistants and rehabilitation support staff
- Participate in interdisciplinary care meetings to support coordinated participant care
- Conduct home visits or educational sessions when indicated in the plan of care
- Maintain timely and accurate documentation in accordance with program policies and regulatory guidelines
- Assist with ordering, maintaining, and tracking durable medical equipment inventory
- Participate in quality improvement initiatives and peer collaboration within the rehabilitation team
Qualifications Required
- Bachelor’s Degree in Physical Therapy
- Graduation from an accredited physical therapy program approved by the American Physical Therapy Association or equivalent organization recognized by CMS
- Active Physical Therapist license in the state of Florida
- At least one year of experience working with older adults or medically complex populations
Preferred
- Two or more years of experience practicing as a Physical Therapist
- Master’s or Doctorate in Physical Therapy
- Bilingual skills are a plus
Compensation and Benefits
- $5,000 sign-on bonus for qualifying candidates
- Hourly pay range of $40 - $52 based on experience
