Role Summary
As a Baylor Home Health RN, you provide skilled, patient-centered nursing support in the home setting, helping clients remain safe, comfortable, and independent. You will complete in-home skilled nursing visits, perform assessments, develop and update care plans, coordinate with physicians and the interdisciplinary team, educate patients and families on disease management, and maintain timely, accurate clinical documentation.
Required qualifications/skills:
- Active Arizona RN license
- Strong clinical judgment, communication, and documentation skills
- Home health experience preferred (not required)
- Ability to manage a flexible field schedule and prioritize effectively

A Typical Shift
Your day is organized around patient needs and clear communication. You’ll review your schedule and notes, connect with the care team as needed, then move through visits with a calm, focused approach. Between homes, you’ll document key updates, confirm next steps with families, and align any changes with provider guidance. You’ll wrap up by completing final charting, returning messages, and preparing for the next set of visits so care stays consistent and on track.
