The Position

As a full-time LPN Home Health, you will support safe, coordinated care for clients receiving services at home while collaborating closely with the clinical team. This role exists to ensure timely clinical follow-through and clear documentation.

Key responsibilities include coordinating client care needs, completing required clinical documentation, communicating updates to the care team, and supporting care plan implementation in alignment with applicable standards.

Required qualifications/skills:
- Current LPN license in good standing
- Home health or related clinical experience preferred
- Strong documentation accuracy and time management
- Clear, professional communication and sound clinical judgment
- Comfort working independently in a fully remote environment

A Typical Workday

Your Monday–Friday day is structured around remote clinical coordination: you start by reviewing messages and updates, then prioritize follow-ups and documentation tasks. Throughout the day, you connect with teammates to clarify next steps, reconcile information in the record, and close out items that keep care moving forward. You’ll pace your workload with steady, detail-focused progress, using secure systems to stay organized and responsive through to end-of-day wrap-up.
