Role Overview
As a Home Health Physical Therapist, you will provide skilled therapy in the home to improve mobility, manage pain, and support safe independence for patients across the East Valley. You will complete PT evaluations, develop and revise individualized plans of care per physician orders, and deliver therapeutic interventions along with caregiver training. You will complete OASIS and all required documentation on time in Homecare Homebase (HCHB), coordinate closely with the interdisciplinary team, supervise PTAs as assigned, and ensure compliance with Medicare Conditions of Participation, agency policy, and Arizona regulations.
Required qualifications:
- Master’s or DPT in Physical Therapy from an accredited program
- Active Arizona PT license; CPR certification
- Strong OASIS and Medicare knowledge; HCHB proficiency
- Valid driver’s license, reliable transportation, and auto insurance

A Typical Workday
Your Monday–Friday schedule is built around in-home visits, with time to review new referrals, confirm appointment windows with patients and caregivers, and travel between locations. Between visits, you’ll update notes in HCHB, respond to care-team messages, and communicate with the ordering physician when patient needs change. You’ll stay aware of home safety factors and equipment needs, then wrap up by finalizing documentation, planning the next day’s route, and ensuring items are completed within required timeframes.
