Licensed Practical Nurse (LPN) -Rehab Unit

JOB TYPE:  Full-time nights, 12-hour shifts, 7p - 7a, every other weekend, must work minimum 1 major & 2 minor holidays, must have 2 years of rehab or Med/Surg experience

HOW YOU’LL CONTRIBUTE

The Licensed Practical Nurse (LPN) provides direct patient care under the supervision of a Registered Nurse, delivering treatments, assisting in assessments, and supporting interdisciplinary care planning in accordance with the patient’s needs.

Essential Functions

  • Administer medications safely, accurately, and based on patient age and clinical condition.
  • Minimize potential for medication errors by following proper medication administration procedures.
  • Assist the RN with initial assessments and patient reassessments per policy.
  • Identify and implement appropriate rehabilitation nursing interventions.
  • Perform timely and accurate Quality Improvement (QI) assessments under RN supervision.
  • Deliver care based on patient response to treatment and update the interdisciplinary plan accordingly.
  • Assess and reassess pain; use appropriate pain management techniques and educate patients and families.
  • Perform all patient care tasks in a manner that promotes patient safety and reduces medical errors.
  • Develop and implement individualized teaching plans based on patient and family learning needs.
  • Perform other duties as assigned.

WHAT WE’RE LOOKING 

Demonstrates strong clinical skills and commitment to safe, patient-centered care.

Ability to work effectively in an interdisciplinary team environment.

Education: Graduate of an accredited Licensed Practical/Vocational Nurse program.

Experience: Minimum six months of Medical/Surgical experience in an acute care setting preferred.

License: Current state licensure as a Licensed Practical/Vocational Nurse.

Certifications: CPR/BLS certification required; IV Certification highly preferred or must be obtained within 6 months.
