Job Summary:Â
Responsible for direct care coverage and supervision in the residential program and the implementation of meaningful day activities to all individuals.Â
Â
Essential Duties/Responsibilities:Â
- Facilitate the implementation of meaningful day in your programÂ
- Ensure daily planning and relaxation meetings are scheduled and occurring:Â
- Ensure entire documents are completed for Mon – Fri from 9 AM to 3 PMÂ
- Ensure that sufficient copies are availableÂ
- Purge information every three months into the black cabinetÂ
- Develop and prepare individualized meaningful day schedules for all individualsÂ
- Ensure daily schedules are being followed and filled out by all first shift staffÂ
- Periodically review schedules for accuracy and completeness, make changes as neededÂ
- Ensure that menus are properly completed and followed.
- Ensure menus are following the correct template and are EO509 compliantÂ
- Post and update menus on the refrigerator or in a location designated by the Program ManagerÂ
- File copies of all menus in the HR bookÂ
- Order necessary supplies for meaningful dayÂ
- Continuously pursue new activities and opportunities to keep schedule freshÂ
- Offer training to staff on meaningful day at each monthly program staff meeting or more often as directed by Program ManagerÂ
- Focus on vocational development and procurement of jobs when possible:Â
Involve individuals in social/recreational activities including:Â
- Socialization and community integrationÂ
- Development of meaningful interpersonal skillsÂ
- Development of habilitation skillsÂ
- Placement transitioningÂ
- Transportation and access of public transportationÂ
- Role ModelingÂ
- Leisure and recreationÂ
- Development of cooking, budgeting and shopping skillsÂ
Involve individuals in Functional Skill Development:Â
- Development of communication skillsÂ
- Personal hygiene and cleanliness of living areaÂ
- Implement Activities of Daily Living. Individual Service Plan Objectives and Behavior Plans.Â
- Provides transportation of individuals to scheduled appointments or social activities.Â
- Record progress notes, behavioral data and log notes for each individual residing in the program.Â
- Tracks data and progress on all identified areas of need.Â
- Ensures the safety of the individuals and general public.Â
- Models’ appropriate behavior for individuals residing in the program.Â
- Engages in the upkeep of the residence.Â
- Ensures adequate program coverage and supervision at all time.Â
- Ensuring duties and responsibilities are carried out in a supportive and professional manner.Â
- Attends appropriate meetings as directedÂ
- Extended standing, sitting, running and lifting as needed to ensure safetyÂ
- Conduct physical restraints as needed.Â

Required Education and Experience:Â
- High school diploma or equivalent.Â
- Position requires criminal history background check.Â
- PABC (physical restraints)Â
- CPR/First AidÂ
- MAP CertifiedÂ
- DDS Licensing and Certification required training Â
Preferred Education and Experience:Â
- Preferred background in Human ServicesÂ
Travel requirements:Â
- Occasional Travel Â
