Job Summary

The Patient Care Coordinator is responsible for the effective coordination and management of patient flow operatives and scheduling to facilitate efficient operation of the department and to provide patients and their families a seamless and exceptional patient care experience.

Minimum Qualifications

Education: Associate Degree required; in lieu of degree four years of progressive medical hospital or office experience in insurance; coding and/or clinical

Licenses/Certifications: (None)

Experience / Knowledge / Skills:

  • Seven (7) years of progressive medical office or hospital related experience  in patient care or healthcare administration preferred

  • Advanced knowledge of anatomy and medical terminology

  • Strong communication, prioritization and organizational skills

  • Ability to work in a team environment; flexibility, adaptability, integrity, and interpersonal skills

  • Effective oral and written communication skills

  • Excellent professional customer service skills

  • Must be able to work independently, in a high stress environment and on multiple tasks simultaneously

Principal  Accountabilities

  • Interprets and coordinates physician’s orders to schedule diagnostic tests, procedures, and/or clinical services.

  • Acts as liaison between the provider, internal and external medical staff in order to relate orders, procedures, and/or care plan to patient.

  • Coordinates and schedules appointments across multiple modalities for patients to ensure seamless delivery of care. Schedules/reschedules patient appointments.

  • Provides patients with pertinent information regarding preparation for services and schedule. Will interview patient/family prior to or on the first visit to verify demographics and financial information.

  • Acts as the primary contact, orients and educates patient and their families by providing information regarding the care plan and facility information.

  • Ensures patient understand every aspect of procedures, test and/or services being provided in a clear and accurate manner.

  • Assists  with resolutions of any issues, complaints or patient/family concerns regarding services.

  • Coordinates an organized and efficient case flow, manages and revises all department scheduling templates, coordinates with clinical staff to work in, add-on patients and coordinate physician time preferences.

  • In collaboration with management will coordinate and develop processes to improve patient throughput in order to create a seamless one-stop experience for the patients will educate clinical staff on new processes.

  • Extracts and collects data to report back to management.

  • Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.

  • Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.
