Job Title/Position: Licensed Practical Nurse

Classification: Non-Exempt

Reports To: Director of Nursing

 

Job Description Summary

The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

 

Essential Job Functions/Responsibilities
  • Provide direct patient care as defined by the State Nurse Practice Act.
  • Implement current nursing practices based on a comprehensive assessment and the established plan of care.
  • Document patient services accurately and promptly to reflect the plan of care.
  • Assess patient and family/caregiver needs and provide education related to the diagnosis and self-management of the care plan.
  • Participate in the coordination of hospice services, reporting identified needs to the interdisciplinary team (e.g., hospice aide, OT, PT, MSW, ST, dietitian, or clinical supervisor).
  • Use equipment and supplies effectively and efficiently.
  • Provide appropriate pain and symptom management, evaluating the patient’s response to treatments and medications.
  • Engage in personal and professional development; participate in quality assessment and performance improvement activities.
  • Perform other duties as assigned by the registered nurse.

Position Qualifications

  • Graduate of an accredited practical nursing or vocational nursing program.
  • Minimum of three years of nursing experience; community health, hospice, or medical-surgical experience preferred.
  • Current LPN/LVN license in the state.
  • Adherence to accepted professional standards and nursing practice.
  • Strong verbal, written, and organizational skills.
  • Valid driver’s license and access to an insured, well-maintained automobile in accordance with state and/or organizational requirements.
  • Current CPR certification.
  • Understanding of the hospice philosophy and the unique needs of individuals who are terminally ill.

Working Conditions & Expectations

  • Works in a variety of environments, including patient homes, assisted living facilities, skilled nursing facilities, and occasional in-office settings.
  • Must maintain reliable transportation, a valid driver’s license, and active auto insurance.
  • Requires flexibility in scheduling to support patient needs, including potential adjustments based on symptom changes, urgent visits, or team coordination.
  • May participate in on-call rotation, after-hours support, or urgent response visits as assigned
  • Collaborates closely with the interdisciplinary hospice team in a fast-paced, emotionally sensitive, and continuously evolving care environment.
  • Expected to uphold hospice philosophy, maintain professional boundaries, and provide compassionate end-of-life care with respect for patient and family needs.

Physical Requirements

  • Must be able to travel frequently throughout the service area and safely enter a variety of patient residences.
  • Requires the ability to sit, stand, walk, bend, kneel, and assist with light patient-care tasks or equipment as needed.
  • Must safely navigate diverse home environments, including stairs, uneven surfaces, cluttered spaces, or limited mobility areas.
  • Ability to use electronic devices (tablets, laptops, mobile phones) to complete documentation in community and home settings.
  • Must be capable of handling emotional and physical demands associated with end-of life care, including prolonged driving, patient interactions, and urgent symptom management situations.
