Position Summary:
The Staff Social Worker provides medically related social services to residents, ensuring
that psychosocial needs are met and that residents attain or maintain their highest
practicable well-being. This role advocates for residents’ rights, supports discharge
planning, and collaborates with interdisciplinary teams.

Key Responsibilities:
• Conduct psychosocial assessments and update care plans based on resident needs.
• Provide counseling and supportive services to residents and families.
• Assist with admission, discharge, and transfer processes, ensuring appropriate
communication with physicians and families.
• Facilitate discharge planning, including coordination with community resources and
services.
• Advocate for resident rights and provide education to staff on psychosocial issues.
• Document social services interventions and progress in resident records.
• Participate in interdisciplinary care conferences and survey preparation.

Skills & Competencies
• Knowledge of long-term care social work practices and regulations.

Required:
• Bachelor’s degree in Social Work or related field preferred.
Preferred:
• Experience in long-term care or healthcare social services.

Physical & Work Requirements
• Ability to move freely throughout the facility and interact with residents and families.
• Occasional lifting of up to 40 lbs and pushing wheelchairs as needed.
