We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Hospice team serving patients in the Tiffin and Fremont communities. This role provides an opportunity to make a meaningful difference by delivering high-quality nursing care and support to patients and families during their hospice journey.
As a Hospice LPN, you will work collaboratively with an interdisciplinary care team to ensure comfort, dignity, and exceptional patient-centered care.
Responsibilities
- Provide skilled nursing care to hospice patients in accordance with the plan of care.
- Monitor patient conditions and report changes to the RN Case Manager and clinical team.
- Administer medications and treatments as ordered.
- Document patient care accurately and timely within the electronic medical record.
- Educate patients and family members regarding disease processes, medications, and comfort measures.
- Collaborate with physicians, nurses, social workers, aides, and other members of the hospice team.
- Support patients and families with compassion, empathy, and professionalism.
- Participate in care conferences and interdisciplinary team meetings.
Qualifications
- Current Ohio Licensed Practical Nurse (LPN) license in good standing.
- Graduate of an accredited practical nursing program.
- Previous hospice, home health, long-term care, or medical-surgical experience preferred.
- Strong assessment, communication, and documentation skills.
- Ability to work independently while maintaining excellent teamwork.
- Valid driver's license, reliable transportation, and acceptable driving record.
Preferred Qualifications
- Hospice or home health experience.
- Experience working with electronic medical records.
- Knowledge of end-of-life and palliative care principles.
