JOB DESCRIPTION SUMMARY:Â
The Intake Coordinator is responsible for managing all aspects of the patient intake process including assisting the members of the intake/admissions team, establishing and maintaining positive relationships with customers. Â

ESSENTIAL JOB FUNCTIONS / RESPONSIBILITIES:Â
• Completes all daily patient referral and intake operations consistent with CMS guidelinesÂ
• Documents communication with patient and providers within the EMRÂ
• Learns, adapts, and implements to new processes as neededÂ
• Audits and monitors referral documents for accuracy and completionÂ
• Completes pre-authorizations and insurance eligibility checks as requiredÂ
• Gathers, tracks, and maintains comprehensive working knowledge of community resources in collaboration with other team membersÂ
• Adheres to HIPAA regulations per Company policy and Federal regulationsÂ
• Consistently represents Signature Healthcare at Home with integrity and professionalism providing excellent customer service to all internal and external customersÂ
• Assists in identifying opportunities for additional/improved services and methods
• Performs other duties and activities as delegated by the Operations Manager, Lead Intake Coordinator, Administrator, and/or Clinical Manager Â

EDUCATION and EXPERIENCE:Â
• High School diploma or equivalentÂ
• Minimum two years’ experience in administrative position in healthcare setting
Â
QUALIFICATIONS and SKILLS:Â
• Knowledge of medical terminologyÂ
• Knowledge of insurance reimbursement processÂ
• Effective data entry, problem-solving, human relations and oral/written communication skillsÂ
• Demonstrates good communications, flexibility and cooperation in performing job responsibilitiesÂ
• Possess a valid driver’s license and insurance in accordance with state and/or organization requirementsÂ
• Basic computer skills and knowledge of office technologyÂ
