The Case Manager will be responsible for developing service plans designed to ensure stable, permanent housing and monitoring of our clients for up to six months. Â Incumbent acts as an advocate and liaison between the client and community service providers to ensure appropriate utilization of resources in meeting the needs of the client

Applicants must have a Bachelor’s degree in the area of human and/or social services or the behavioral sciences. Education and/or verifiable work experience with at-risk populations is required (homeless, veterans, behavioral health, drug/alcohol addiction, employment, youth, residential).
