The Role

We are looking for a compassionate Hospice Social Worker to join our team. In this role, you won't just manage cases; you’ll hold hands, navigate complex emotions, and provide the "soft landing" every family needs during their hardest moments.

What you’ll do:

• Provide emotional support and crisis intervention.
• Assist families with end-of-life planning and resource navigation.
• Be the bridge between the clinical team and the heart of the home.
