Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas. Physician Services Healthcare practice that provides personalized primary care services for older adults and individuals living in rehabilitation centers, skilled nursing facilities, assisted living communities, and independent living settings. The organization focuses on delivering compassionate, patient-centered medical care directly within residential and long-term care facilities to improve comfort, accessibility, and continuity of care. Their providers specialize in coordinating care with hospice, home health, and rehabilitation teams to support patients with aging-related conditions, chronic illnesses, recovery needs, and dementia care. The group emphasizes individualized treatment plans, collaborative care coordination, and improving quality of life for patients and their families. Nurse Practitioner The Nurse Practitioner provides comprehensive, patient-centered primary care to older adults and individuals residing in rehabilitation centers, skilled nursing facilities, assisted living communities, and independent living settings. The role focuses on assessing and managing acute and chronic health conditions, monitoring changes in patients’ health, developing individualized treatment plans, and supporting patients throughout recovery and long-term care. The Nurse Practitioner collaborates closely with physicians, nurses, caregivers, families, hospice, home health, rehabilitation, and other healthcare professionals to ensure coordinated and continuous care. Responsibilities may include performing physical assessments, ordering and interpreting diagnostic tests, managing medications, addressing acute illnesses and chronic disease, providing dementia and geriatric care, and helping patients and families understand treatment options and goals of care. By providing medical care directly within residential and long-term care settings, the Nurse Practitioner helps improve accessibility, continuity of care, comfort, and overall quality of life while reducing unnecessary hospital visits and supporting patients through changes in their health and functional needs.
